About the Role
Job Overview

Provide clinical care to patients of the pharmacy. Assist the Oncology Pharmacists in the management of patients taking oral chemotherapy. 

 

  • Provide appropriate clinical care including patient counseling and prospective drug use evaluation to all patients receiving their medication therapy from the Pharmacy.
  • Perform initial patient evaluation as it pertains to the prescribed oncolytic therapies. Collect, analyze, interpret, and evaluate appropriate data relative to medication therapy.   
  • Evaluate the significance of signs and symptoms relative to disease processes and makes recommendations for the modification of drug therapy.
  • Evaluate the significance and clinical importance of adverse reactions and complications associated with drug therapy.
  • Monitor patient records for potential drug interactions as well as patient allergies.
  • Provide counseling and answer questions from patients, family and health care professionals concerning drug therapy.
  • Communicate with patients’ care teams to manage adverse effects, supportive care needs, and changes to oral oncolytic therapy
  • Identify and resolve patient specific medication related problems and issues in the patient’s best interest.
  • Ensure dispensing of prescriptions in accordance with all legal requirements, regulatory agencies, third party contracts, and Fairview Pharmacy Services Policy and Procedures.
  • Utilize all technologies to ensure accuracy and efficiency in the dispensing process.

  • Provides patient education based on as assessment of learning needs of patient/care giver.

  • Completes all required learning relevant to the role 
  • Complies with and maintains knowledge of all relevant laws, regulation, policies, procedures, and standards. 
  • Fosters a culture of improvement, efficiency, and innovative thinking. 
  • Performs other duties as assigned 

 

Minimum Education: 

  • Enrolled as a Student at an ACPE accredited school of Pharmacy beginning Fall.  
  • Finished First Year of Pharmacy School 

Preferred: 

  • Enrolled as a Student at the University of Minnesota School of Pharmacy.  
  • 2nd year pharmacy student transitioning to 3rd year after this spring.

Required License/Certification/Registration: 

  • Current Minnesota Pharmacy Intern License 

Additional Requirements 

  • Excellent customer service skills, proficient in English language, capable of solving problems effectively, ability to follow policies and procedures, and competent mathematical skills. Previous pharmacy experience preferred.
