About the Role

POSITION SUMMARY


This position is with the University of Hawaii at Hilo Daniel K. Inouye College of

Pharmacy (DKICP) Medication Therapy Management (MTM) Center. The Medication Therapy Management (MTM) APPE rotation provides student pharmacists with a focused, patient-centered learning experience centered around the optimization of medication use. During this rotation, students will work under the supervision of a pharmacist to assess, identify, and provide recommendations to address medication-related problems, with the goal of improving therapeutic outcomes and enhancing patient safety.


Students will complete medication reconciliation and assist with completing comprehensive medication reviews (CMRs) via telephone visits with patients and communicate recommendations with providers.


In addition, students will also get experience in the operations of a service/program focusing on medication management and will help with quality improvement projects to ensure the organization is meeting goals/objectives for the organization, have streamlined workflows and meet the scope of work and performance guarantees of the companies that we have contracts with. The student will gain experience with using a nationally used platform for

completing medication reviews.


REPRESENTATIVE DUTIES: 


  • Work collaboratively to maximize individual member/patient outcomes.

  • Coordinate and disseminate member/patient communications including welcome letters, member/patient resources, and member/patient and provider letters.

  • Contacts members/patients to explain the MTM program and schedules appointments to complete the beneficiary’s medication reviews.

  • Collects and screens inbound communication (phone calls, voicemail messages, facsimiles, mail).

  • Assists with providing patient-centered MTM services for the purpose of optimizing medication therapy to improve outcomes.

  • Assist in providing MTM services including, but not limited to, medication reconciliation, comprehensive medication reviews, targeted interventions, adherence monitoring interventions, cost-effective interventions, and transitions in care to help achieve these quality outcomes.

  • Appropriately documents patient encounters and interventions related to the MTM services provided.

  • Maintains competence required for current position and assists in training of new employees and students.

  • Participates in continuous performance improvement and regulatory compliance efforts.

  • Maintains accountability in quality improvement efforts and subsequent monitoring of process and results.

  • Participates in required operational meetings and training sessions as necessary.

  • Performs other duties as assigned or required.

  • Maintains an attitude that promotes and fosters an environment of safety.

  • Complies with all legal requirements and company policies.

  • Performs all other duties as assigned.


QUALIFICATION REQUIREMENTS

Education/Training

Minimum:

  • Must be currently enrolled at the University of Hawaii at Hilo, Daniel K. Inouye College of Pharmacy, Doctorate of Pharmacy Program and assigned to the MTM program for the Advanced Pharmacy Practice Experience.

  • Valid and good standing Hawaii State Pharmacy Intern License


Skills Knowledge

Minimum: 

  • Strong communication skills (both oral and written).

  • Good organizational and oversight skills.

  • Knowledge of pharmacy/medicine and common operating procedures/practices.

  • Strong technical (medical) writing ability.

  • Knowledge/Experience, spreadsheets, shared drives, etc.
