MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Must possess current licensure as required by state board where services will be provided:

MD: Pharmacist or Pharmacy Intern license through Maryland Board of Pharmacy

OH: Pharmacist or Pharmacy Intern license through Ohio Board of Pharmacy

PA: Pharmacist or Pharmacy Intern license through Pennsylvania Board of Pharmacy

WV: Pharmacist or Pharmacy Intern license through West Virginia Board of Pharmacy

NOTE: Residents working under intern license must obtain Pharmacist license within 120 days of hire. If Pharmacist license is not obtained within 120 days of the resident's start date, the Resident Pharmacy Director will notify the Resident they are not eligible to receive a Certificate of completion, and their Resident contract will be terminated.

NOTE:  Residency programs may require a shorter timeframe to obtain permanent licensure than what is stated in the residency program manual due to staffing and rotational requirements. 

2. Obtain certification in Basic Life Support within 30 days of hire date.
