Reporting to the Director of Quality and Healthcare Innovation, the Intern - Population Health Intern works within Community Healthcare Network’s population health department to ensure patients receive quality medical care that aligns with our quality indicators. This internship provides valuable hands-on experience in care coordination and quality improvement efforts that strive to improve patient health outcomes. This fully remote position works closely with the interdisciplinary team and serves as a telephone liaison between patients and their care team. 


DUTIES AND RESPONSIBILITIES: 

Essential Functions: 

  1. Contacts patients daily to facilitate patient compliance and quality indicators, such as required annual health screenings, and schedules appointments within appropriate time frames.
  2. Extracts data from internal and external data platforms to compile patient outreach lists, as directed.
  3. Maintains consistent communication with patients, clinicians, and the care team to ensure patients’ needs are addressed in a timely manner.
  4. Receives incoming emails and faxes from laboratory, physician offices, hospitals, and diagnostic centers and assigns to appropriate parties. 
  5. Scans incoming medical documents, review images for quality and upload to the patients’ medical record.
  6. Reviews individual clinical records for the presence of adequate and required documentation, using chart review tools
  7. Prepares weekly reports documenting the outcomes of outreach activities.
  8. Participates in performance improvement activities, as assigned.
  9. Facilitates related services for health center patients, as assigned.
  10. Performs other related duties, as assigned.


WHAT WE LOOK FOR:

 

  • High School diploma or satisfactory equivalent 
  • Demonstrated interest or experience in healthcare, medical records, or quality assurance
  • Strong communication skills, attention to detail, and organizational abilities
  • Comfortable working independently in a remote environment
  • Proficiency in Microsoft Office and basic data entry
