Family Service Toronto (FST) helps people face a wide variety of life challenges. Program Summary
The Options program operates within an anti-oppressive framework and is committed to building inclusive communities by working in partnership with individuals with intellectual/developmental disabilities, and their families, friends and communities, community organizations, developmental service agencies, generic agencies, and relevant systemic and community networks.
The mandate of this program is to empower children and youth with a developmental disability and their families through interventions that support person-directed, goal-oriented planning, service coordination, circles of care facilitation, self-advocacy, and peer support groups.
The Opportunity!
Children's Community Resource Facilitator provides information, referrals, and coordination of service for children with developmental disabilities and their families. Goal-focused, time limited, service plans are developed and reviewed regularly with clients. Information is provided to help clients make informed choices; to access both traditional and non-traditional supports in the community.
Responsibilities:
- Assist clients in identifying their strengths and needs.
- Work with clients to develop goal oriented, time limited-service plan.
- Support clients in the implementation of the plan.
- Support clients to locate, access and evaluate appropriate supports and community resources.
- Monitor and reviews service plan effectiveness with clients/families on regular basis.
- Collaborates with service providers and community resources as needed.
- Support with regular and timely recording of case notes and clients service data as required by funders and FST policy.
Regular supervision and training will be provided to students to obtain relevant knowledge and skills to perform mentioned responsibilities.
Qualifications:
Students must possess:
- Enrolled in a College or Bachelor program related to social work.
- Good documentation and organizational skills.
- Critical thinking skills, open mindedness and willingness to learn and self reflect.
- Commitment to ideals of equity, diversion and inclusion.
- Successful Criminal Record and Vulnerable Sector Check required.
Skills that would be an asset:
- Knowledge of the developmental services sector and community- based resources in Toronto.
- Experience working with individuals with a developmental/intellectual disability and/or in the social service sector.
- Good computer skills (Microsoft Word, Excel, PowerPoint and Access).
- Ability to work within a fast-paced environment.
