Pharmacist Intern - Flex/PRN

Under the general direction of a Pharmacist, Pharmacist Interns will work with Licensed Pharmacists to provide care for patients in the areas of operational support and clinical services. Job duties are delineated by professional year of Pharmacy School. Essential functions and responsibilities will include central operational Pharmacy activities and including clinical responsibilities, such as medication reconciliation and patient counseling.

1. ESSENTIAL FUNCTIONS, DUTIES AND RESPONSIBILITIES BY PROFESSIONAL YEAR:
   1. Generally, the skill set of the first and second year Pharmacist Intern centers around medication reconciliation, however, the second year Student may have some additional limited clinical responsibilities under the supervision of the Pharmacist depending on the skills set provided by each year of school. Third and fourth year Pharmacist Interns will have more clinical responsibilities and assignments that advance in scope as the Intern’s education and clinical training develop. The intern may be assigned to medication reconciliation, inventory or as an immunizer if they have completed EMRMC or professional training courses required.
2. FIRST YEAR PHARMACIST INTERN:

1. Preparation and labeling of non-sterile oral and topical products under the supervision of a Pharmacist.
2. Preparation and labeling of sterile intravenous dosage forms as ordered by the physician under the supervision of the Pharmacist.
3. Restocks the automated dispensing cabinets when prompted by low pars and fills cart-fill to be delivered to the nursing units.
4. Responsible for checking cart-fill (EMRMC only) and restock for certified Pharmacy Technicians.
5. Maintains records of medication and narcotic distribution while adhering to Federal and State Laws regarding distribution, dispensing, and destruction of controlled substances.
6. Investigates and reconciles discrepancies of controlled substances through the use of the automated dispensing cabinet.
7. Assists the Pharmacist in maintaining an adequate inventory of medications through ordering and receiving inventory if needed.
8. Disposes of hazardous drug waste appropriately per the Hazardous Waste Policy and documents all waste on the Hazardous Waste Manifests.
9. Packages and labels unit dose medications, including bar codes, to facilitate patient safety and meet all Federal and State requirements.
10. Triages phone calls knowing when to refer to the Pharmacist or handle independently.
11. Completes a detailed transaction report for billing medications issued from Central Pharmacy Inventory to other Pharmacies, vendors, or departments.
12. Screens for appropriate storage and security of medications in the Pharmacy and other departments through regular surveys.
13. Delivers medications to and from the nursing stations and other departments as needed while maintaining appropriate security of medications per Federal and State legislative requirements. This position is authorized to gain entry into medication rooms/storage areas and have access to medications within the scope of their essential job description.
14. Delivers filled prescriptions from the Outpatient Pharmacy to the patient’s bedside as part of the Pharmacy Concierge Service (EMRMC only).
15. Medication reconciliation for new patients admitted through the Emergency department or overnight with no medication history completed. 

1. SECOND YEAR PHARMACIST INTERN: In addition to the tasks listed above, second year Pharmacist Interns may have additional responsibilities including, but not limited to:
   1. Replaces drug items and supplies on crash carts and other drug storage carts/boxes.
   2. Compares the medications that are ordered, administered, or dispensed in the hospital to the patient’s historical medication list and flagging follow up for omissions, duplications, or other potentially harmful discrepancies for Pharmacist review.
   3. Contributes to the operational effectiveness of the Pharmacy by evaluating drug regimens for potential IV to PO conversions per protocol and flagging follow up for the Pharmacist if the patient needs review.
   4. Review of medication profiles to ensure that patients have DVT prophylaxis ordered and flagging follow up for the Pharmacist if the patient needs review.
   5. Educates patients per the Anticoagulation policy and documents appropriately in the patient’s electronic medical record.
   6. Immunizer for vaccinations with documentation of completed training courses from College of Pharmacy or CDC.

1. THIRD YEAR PHARMACIST INTERNS: In addition to the tasks listed above, third year Pharmacist Interns may have additional responsibilities including, but not limited to:
   1. Chemotherapy admixture.
   2. Delta patient education on medications and post discharge calls and documentation.
   3. Renal dosing and adjustments.
   4. Patient counseling on discharge medications and concierge prescriptions.
   5. Adverse event reporting.
   6. Performance Improvement data collection and reporting.
   7. Drug monograph preparation for formulary approval at P&T.

1. FORTH YEAR PHARMACIST INTERN: In addition to the tasks listed above, fourth year Pharmacist Interns may have additional responsibilities including, but not limited to:
   1. Kinetic dosing consults.
   2. Anticoagulation Dosing and Monitoring.
   3. Antimicrobial stewardship reviews for restricted antibiotics.
   4. Completion of Medication Use Evaluation and presentation to P&T Committee.
   5. Training new Interns, Students or Technicians.
   6. ED shifts for medication reconciliation and code observations. 

Must have sufficient experience in a hospital Pharmacy to successfully fulfill the responsibilities of the position.
